NEW Good Faith Estimate & Consent Forms:

Under the Public Health Service Act, health care providers need to give clients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services. At Genuine Connections Psychotherapy, a new Good Faith Estimate will be issued in January of the calendar year for continuing clients and will reflect any new rate increases. Additionally, all clients will be expected to complete all new policy forms, consent forms, financial agreements etc…for the new calendar year.
